5/23/2023 EnchantaVaughn Approved 1.Behavior: Anthony greeted QMHP-C upon arrival to the home. Upon arrival to the home Anthony appeared to be in a content mood evident by his facial expressions such as a smile on the face. QMHP-C informed Anthony this session was going to continue to focus on controlling his anger and assisting the mother with being able to implement and reinforce consequences for behaviors being presented within the home for both good behavior and negative behavior. 2. Behavior: Anthony presented normal and cooperative behaviors throughout the session needing no verbal prompts from QMHP-C or his guardian. 1.Intervention: QMHP-C facilitated a discussion focused on assisting the parent with maintaining control of Anthony behavior. QMHP-C reexamined with the parent the acceptable and unacceptable behavioral standards for Anthony previously set in place. QMHP-C reminded the parent that the responsibility for enforcement of the rules of the home was hers. QMHP-C reminded the parent that she has to be firm in her enforcement of the rules of the home. QMHP-C reminded Anthony that there is a standard of behavior within the home that he is expected to live up to. QMHP-C informed Anthony that level of expected behavior extends to the community and home setting. QMHP-C facilitated a discussion on the previously assigned homework assignment "Switching from Defense to Offense" exercise from the Brief Child Therapy Homework Planner (Jongsma, Peterson, and Mclnnis). QMHP-C informed the parent that in order to change Anthony behavior patterns she must move to offense where she can gain control and be an effective parent. 2. Intervention: QMHP-C informed the parent that any effective offense must have a game plan that is specific, focused and consistent to be able to be successful. QMHP-C suggested to the parent to use this exercise to come up with different solutions to implement to change past patterns of unsuccessful conflict resolution, disrespect, and uncooperativeness with Anthony. QMHP-C reexamined each of the five steps in the exercise with the parent in detail (identify problematic behaviors, describe desire positive behaviors, rewards for positive behaviors, punishments for problem behaviors, and plan ahead for anticipated problems). 1.Response: Anthony was receptive to QMHP-C's suggestions for today's session. Anthony and parent were very receptive and responsive in today’s session. Anthony stated that he is aware of the level of behavior that he is expected to live up to. Anthony stated that he does his best to follow the rules and act accordingly. The parent voiced that she believes that this exercise was very helpful in finding news ways to manage Anthony behavior. 2. Response: The parent stated that whenever the siblings get to playing she anticipates that there will be a situation that she will have to deal with. Anthony stated that he was happy with todays session. Anthony stated tht there are still allot of things he needs to improve on. Anthony thanked QMHP-C for conducting a session today and concluded todays session.
